SBA Disaster Loan Application Process
- To complete the application online, register Username and Password on the SBA disaster loan assistance website, here.
- Applying online is the most expeditious
- If you prefer to complete the loan application on paper, blank forms can be found here, or listed below.
- Mail To: U.S. Small Business Administration, Processing and Disbursement Center, 14925 Kingsport Road, Fort Worth, TX 76155-2243
Sole Proprietor information needed to complete the forms:
- Personal Information (Date of Birth, Social Security Number)
- Annual Income
- Personal Assets and Assets related to business
- Personal Liabilities
- Detail Loan Information (Car, Student Loan, Mortgage)
- Original Balance, Current Balance, Monthly Payment
- Retirement Account Information
- Representative Fee Information (Only if you paid someone to complete application)
